Community Link Worker: Create Pathways to Wellbeing

NHS Lanarkshire is seeking a Community Link Worker to support GP practice patients with person‑centred care. You will help patients identify issues affecting wellbeing and work with a multi‑disciplinary team to overcome barriers.

The role runs 36 hours over Monday to Friday, 08:00–18:00 (to be confirmed) and is fixed‑term until 30‑09‑2027. You will collaborate with coordinators, empower patients to access training and community activities, and promote the CLW role within Primary Care Teams.
