GP with Special Interest in Frailty and Care Homes
6-8 sessions per week – fixed term for 24 months
The post-holder will deliver high-quality, proactive, and coordinated care to patients living with moderate-to-severe frailty and act as the clinical lead for frailty across the PCN (5 member practices). The role focuses on shifting care from reactive crisis management to proactive, anticipatory care planning.
Key objectives include conducting Comprehensive Geriatric Assessments (CGAs), reducing avoidable hospital admissions, and supporting patients to remain independent in their preferred place of care.
The role is designed to standardise frailty care across the network, manage the PCN-wide PNG 10,11 and care home cohorts as identified through the Johns Hopkins risk stratification tool.
