Community Health Support Worker – Home Visits

Wight Healthcare in Portsmouth is seeking a Community Healthcare Support Worker to join our Integrated Care Team, delivering community-based services to a GP Practice population and undertaking nursing tasks within agreed parameters.

The role involves home visiting, lone working, supporting admission avoidance and early discharge, and promoting independence for patients, including those with long-term conditions and older people with mental health problems.
