Social Worker – Drive Care & Support Plans

Oldham Council is seeking a dedicated social care professional to manage a caseload, undertake assessments and develop Care & Support Plans under supervision. The role involves working across sites with some home working options.

You will deliver and update Care & Support Plans using person-centred approaches, collaborate with health and voluntary sector partners, and support service users and families to make informed choices.
