Contract Hospital Social Worker: Discharge & Care Transfer

Service Care Solutions Ltd is seeking an Agency Experienced Hospital Social Worker to join the Transfer of Care Hub at our client Hospital on a temporary contract. This fast-paced role focuses on safe, timely discharges with hybrid working (three days in the office).
