Job Description:
Northumberland County Council is seeking a Social Work Support Assistant to join the Home Safe Hospital Discharge Service at Northumbria Specialist Emergency Care Hospital in Cramlington. You will work to assess needs, develop care plans and support safe hospital discharges, in collaboration with families, carers and external services.
The role is permanent, full-time (37 hours per week). Salary is £28,598–£31,022 per annum, with a flexi scheme and access to the Local Government Pension Scheme.
