Reintegration Peer Support Worker – Remote Options

The Forward Trust in Buckinghamshire is seeking a RECONNECT Peer Support Worker to help people preparing to leave custody connect with health, housing and wellbeing services. You will work with clients across HMP Bedford, HMP Woodhill, Yarls Wood IRC and surrounding communities, with some remote working.

You will complete assessments, co‑produce care plans and provide up to 6 months post‑release support, building partnerships with external agencies and ensuring safe, person‑centred service
