Community Health Nurse – Field-Based Care

Epsom and St Helier University Hospitals NHS Trust invites applications for a Band 5 Community Nurse to work across Epsom PCN. If you are passionate about caring for people in their own homes and communities, we’d love to hear from you.

You will assess care needs, develop and evaluate holistic plans, mentor colleagues, and work with the multidisciplinary team. A current UK driving licence and access to a vehicle are essential.
