Band 5 Community Nurse

Band 5 Community Nurse – Kent Science Park, Sittingbourne

We are looking for a motivated and caring Registered Nurse to join our community nursing team in the Kent area. The role works 7‑day service from 08:00 to 20:00 and focuses on supporting patients and their families throughout their rehabilitation journey.

Responsibilities

• Provide safe, high‑quality nursing care to patients and their families.
• Conduct thorough assessments and develop individual care plans.
• Coordinate with multi‑disciplinary teams to ensure continuity of care.
• Maintain accurate and timely clinical records.
• Communicate effectively with patients, families and colleagues.
• Implement best practice, contribute to service improvement and audit activities.

Qualifications & Experience

• Registered General Nurse with a current NHS license to practice.
• Band 5 experience or equivalent in a community setting preferred.
• Strong communication, organisational and IT skills.
• Proficient with Microsoft Office (Outlook, Word, Excel).
• Willingness to travel to and within the service area (minimum driving licence and access to a vehicle is essential).
