Community Reablement Assistant Level 2 – Enable Independence

NHS Hampshire is seeking a Community Reablement Assistant (Level 2) to join our Reablement Team in Hampshire. You will support adults over 18 to maximise independence and remain at home, delivering person-centred care during initial home visits and reablement interventions.

You’ll conduct risk assessments, help set goals, and monitor progress, while coaching and mentoring other assistants and supporting Team Leaders, Occupational Therapists and Sensory Officers. Competitive benefits apply.
