Overview

To work within the Community Integrated Service to support and assist in the provision of holistic nursing care and support to patients, their families, and carers.

Responsibilities

  • Coordinate and participate in specialist care such as wound care, Doppler testing, phlebotomy, bladder screening and other clinical activities under the supervision of Registered Nurses.
  • Undertake diagnostic tests requested by District Nurses, Community Matron or GPs (e.g. chronic disease monitoring programmes), including home visits and coordination of repeat or regular testing for residents in residential homes.
  • Undertake clinical interventions delegated by a senior team member.
  • Participate in the assessment process of care needs and implementation of programmes of holistic care within set standards; support and educate families/carers where appropriate.
  • Maintain clear and contemporary records, signed and contemporaneous, following Surrey Downs Health and Care procedures; take and record telephone messages, respond and liaise appropriately.
  • Manage stationery and documentation supplies; record mileage and annual leave on staff databases.
  • Work as part of the team, participating in procedures and duties in accordance with agreed standards; maintain regular contact with senior team members and communicate all relevant issues.
  • Ensure privacy and dignity of patients; liaise with senior team member and collaborate with other professionals and agencies; act upon procedures protecting vulnerable individuals.
  • Participate in Surrey Downs Health and Care Governance Strategy and Risk Management Strategy.

Qualifications

  • Administration, clerical and IT skills.
  • Ability to provide quality, patient‑responsive care and work in partnership with patients and other agencies.
  • Good written and verbal communication skills.
  • Ability to organise own time effectively.
  • Ability to work effectively as part of a team, valuing contributions from team members.
  • Ability to work independently following spoken or written instructions.
  • Core nursing/clinical skills such as wound care, venepuncture, diagnostic tests.
  • Willingness to move base temporarily or permanently within Surrey Downs Health and Care localities as required.
