Discharge Planning Social Work Assistant (12-Week)

Centreforqualities is seeking a Social Work Assistant for a 12-week temporary post in the Social Work Hospital Discharge Team (West). You will support adults with physical and/or mental health needs, assisting with discharge planning and coordinating with social workers to promote person-centred outcomes.

The role involves collaboration with families and carers, travel within Fife, and requires PVG Level 2 clearance. Training aligned to National Induction Framework is provided.
