Reablement Home Care Team Lead

Cardiff Council’s Community Resource team Home Care (CRT) forms part of the Independent Living and Community Care Service and works with vulnerable adults to support them to live independently at home and stay connected to their communities.

This role involves line managing CRT Home Care staff, coordinating with health, social care and third sector partners, and developing pathways to support hospital discharge and prevent admissions.
