Frailty Nurse Team Lead – Community Care & MDT Impact

NHS in York is seeking an experienced registered nurse to join the Community Frailty Hub within Integrated Neighbourhood Teams. You will lead a dynamic team of nurses and support staff, delivering proactive, nurse-led geriatric assessments and coordinated care in the community.

You will work as part of the Complex Frailty INT, collaborating with primary care and specialists to stabilise health, prevent crises, and support people to live well at home.
