Clinical Pharmacist: Medicines Optimisation & Patient Care (GP)

Cardiff and Vale University Health Board is seeking an enthusiastic Clinical Pharmacist to join a friendly multidisciplinary team within a GP practice in a patient-centred role. You will deliver high-quality pharmaceutical care, provide medicines optimisation, and support safe prescribing for registered patients.

You will collaborate with GPs, nurses and pharmacy colleagues to improve patient outcomes, manage prescription queries, and contribute to service improvement and QA projects.
