Community Team Leader (Female Only)

Location: Hull (Packages of Care) & Doncaster Office. Salary: £30,900 per annum + £3,000 Car Allowance – Monthly pay. Hours: 36 hours per week with 12 hours on shift and 24 hours Management and Admin.

About the Role

We are looking for a compassionate, experienced, and motivated Community Team Leader to lead and support our teams across a range of community-based care packages in Hull, while also working from our Doncaster office. This is an exciting opportunity for an individual with strong leadership skills and a passion for delivering outstanding person-centred care to people with complex health and mental health needs.

Key Responsibilities

  • Lead and supervise Support Workers across multiple care packages.
  • Ensure high-quality, safe, and person-centred care delivery.
  • Conduct observations, competency assessments, and staff support sessions.
  • Monitor care records, documentation, and service quality.
  • Support recruitment, induction, training, and development of staff.
  • Ensure compliance with CQC regulations and safeguarding requirements.
  • Work collaboratively with families, professionals, and multidisciplinary teams.
  • Participate in the on-call and escalation process.
  • Support the continuous improvement of services.
  • Promote positive outcomes and independence for people we support.

Essential Requirements

  • Female applicant (Genuine Occupational Requirement applies under the Equality Act 2010).
  • Full UK Driving Licence.
  • Access to your own vehicle.
  • Excellent communication and interpersonal skills.
  • Flexible approach to working hours and service requirements.
  • Experience working within health and social care settings.
  • Experience supervising, mentoring, or leading care teams.
  • Understanding of person-centred care and risk management.
  • Understanding of CQC standards and safeguarding frameworks.
  • Strong understanding of care plans and outcome-focused support.
