Overview

The post-holder will be a member of the Neighbourhood Nursing (District Nursing) Team and will participate in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions. Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion. The care will be provided in a range of settings including patients’ homes, residential homes, nurse led clinics and GP practices.

Responsibilities

  • Holistic assessment, planning, implementation and evaluation of a plan of care for patients with long term conditions, tissue viability, continence, palliative care, and promoting/facilitating patient independence.
  • Assist in delivery of National Service Frameworks.
  • Provide public health and health promotion programmes to improve health and reduce inequalities.
  • Maintain contemporaneous written and computerised records.

Qualifications / Education

  • Registered Nurse (General Adult).
  • Education to 1st degree level or equivalent.
  • Clinical specific post registration course e.g. Wound management, diabetes, leg ulcer care, continence care and assessment.

Experience

  • Extensive experience in the care of people with long term conditions in a community or primary healthcare setting.
  • Experience in care of people with long term conditions.

Competencies and Skills

  • Ability to assess, plan, implement & evaluate programmes of care.
  • Ability to demonstrate evidence-based care and decision making that is responsive to the needs of clients and carers and negotiated in partnership with them.
  • Competent in basic life support.
  • Ability to work as an autonomous practitioner being responsible and accountable for choices made.
  • Ability to work flexibly and respond to changing demands in workload.
  • Ability to communicate effectively at all levels within the organisation, with patients, relatives and their carers and other agencies.
  • Ability to write detailed reports, maintain contemporaneous patient records and other documents that exhibit fluency and accuracy.
  • Skilled in the provision of hands on direct patient care, including safe handling of patients and equipment.

Regulatory Evidence

  • Current NMC Registration.
  • Evidence of post registration education and training.
  • Practise assessor/supervisor or old mentorship qualification or SSSAP.
