To be a Mental Health Nurse or Mental Health Social Worker within the Op Courage service to veterans and “service leavers” with a six month discharge date from Military, within the North West of England.

As part of the OpCourage North collaborative, assist in triage/ screening referrals for the collaborative, alongside specifically NW clients.

The post consists of hybrid working between office, home base, online/ virtual appointments, face to face and community settings.

Responsibilities

  • Participation in a Duty rota including liaising with referrers and other services, providing support to members of the Multi-Disciplinary Team in relation to risk management and safeguarding issues
  • Case management of veterans, clinical interventions and signposting/ liaising with local services
  • Screening and telephone assessment duties including liaison with referrers and other service providers
  • Provide a thorough psycho-social assessment of the client (aged 18+), risk assessment, safety plan and care plan
  • Deliver individualised interventions based on care plan
  • Manage own caseload by delivering brief interventions and allocating new clients
  • Identify and develop collaborative care plans and manage referrals to safeguarding and other services
  • Maintain electronic records using PC‑MIS and ensure documentation meets Trust policies
  • Act in accordance with NMC Code of Conduct and participate in supervision and MDT meetings
  • Deliver services across the North West, including home visits, as required

Main duties

Senior mental health practitioners would be expected to carry out initial face to face mental health, social care and risk assessments using the OpCourage standard assessment, case manage an assigned group of clients requiring ongoing monitoring of mental health and risk management, provide emotional and practical support, and deliver evidence based interventions supported by the psychological team.

Person specification

Skills and Abilities

  • Able to conduct thorough psycho-social assessments and develop collaborative care plans
  • Skill in delivering therapeutic interventions in a community setting
  • Ability to build professional relationships within multidisciplinary team
  • Working knowledge of MoD and other agencies (SMS, CMHTs, IAPTS, GPs, voluntary agencies)
  • Excellent written and verbal communication skills
  • Proactive risk assessment and management
  • Excellent organisational skills
  • Case management of clients and delivery of brief interventions
  • Screening and telephone assessment skills, mentoring of student nurses

Knowledge

  • Primary Care Mental Health and brief interventions
  • Understanding of mental health needs of Military Veterans and families/carers
  • Risk management knowledge
  • Current issues and legislation within Primary Care Mental Health (IAPT model)
  • Safeguarding Children and Vulnerable adults
  • Knowledge of NICE approved evidence based therapies
  • Contribution to service CPD

Education / Qualifications

  • RMN or MHSW
  • Post graduate diploma level or higher
  • Certificate / Diploma in CBT or counselling
  • CPD in Primary Care Mental Health

Experience

  • Case management of clients with complex mental health difficulties
  • Experience in a community NHS setting
  • Experience in multidisciplinary team
  • Experience in armed forces communities or related service charities
  • Management of community client caseload
  • Delivery of therapeutic interventions in community setting
  • Group work experience
  • Mentor/ assessor for student nurses
  • Experience with drug/alcohol problems and recovery models
