Overview
You will have the opportunity to make profound improvements in how crisis care is experienced by young people and those who care for them. You will be working closely with our inpatient colleagues and have responsibility for managing the Single Point of Access for all inpatient referrals for the GM population of 13‑18 year old young people who are being considered for inpatient care.
Responsibilities
Co‑ordinate and manage the Single Point of Access for all inpatient referrals, working with CYP with a variety of mental health difficulties and diagnoses such as Psychosis, Bipolar Disorder, Emotional dysregulationSchizophrenia, Depression/anxiety, LD, ASD, ADHD, PTSD, Conduct disorder and Attachment difficulties.
Collaborate with external services such as social care, CAMHS, FCAMHS, CMHT, CEDS, EIT, HTT to ensure correct pathways are in place and to support our CYP’s discharge from hospital.
Work closely with the Greater Manchester Assessment and Inreach Centre team to reduce the dependency on inpatient care and support CYP in a timely manner at home.
Qualifications
Relevant experience working with children and young people with mental health difficulties and evidence of skills in the assessment of risk.
Experience of working across agencies and developing holistic care plans.
Knowledge of relevant legislation is preferred and a sound understanding of safeguarding processes is essential.
Enthusiastic, proactive, engaged in learning and development, and passionate about working within crisis care.
