We are looking to appoint a Transitions Social Worker to join our newly developed Preparation for Adulthood (PfA) Service within SEND Central in York. This role sits within Adults Social Care and works at the interface between children’s and adult services, supporting young people with SEND to move into adulthood in a timely, planned and person‑centred way, in line with York’s Preparation for Adulthood vision.

As part of a multi‑agency team, you will work closely with Children’s Social Care, SEND, education, health and other partners to deliver coordinated and consistent transition planning, starting early and focused on strength‑based outcomes.

Responsibilities

  • Lead on Care Act assessments and support planning
  • Manage a complex caseload
  • Work collaboratively across agencies to support effective, outcome‑focused transitions
  • Contribute to early planning (from Year 9 onwards)
  • Promote strengths‑based practice, supporting independence, employment, community inclusion and positive adult lives

Qualifications

  • Qualified Social Worker, registered with Social Work England
  • Experience of complex practice and multi‑agency working
  • Strong knowledge of the Care Act and Mental Capacity Act
  • Confident in decision‑making and managing complexity
  • Committed to supporting young people to achieve independence and positive outcomes

Interview Assessment

Candidates will be required to demonstrate competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C1.
