Job Description:
AWP (Avon and Wiltshire Mental Health Partnership NHS Trust) seeks a Consultant Physician to support two teams: West Wilts CMHT and the Dementia Service, delivering medical input for around 15,000 older adults across Wiltshire communities including Bradford-on-Avon, Melksham, Corsham and parts of Trowbridge.
The post involves clinical leadership, MDT collaboration, community clinics, home visits and CPA involvement, with a focus on integrated, patient-centred care.
