Advanced Care Practitioner: Lead in Primary Care

Greenwich Health in London is seeking an experienced Advanced Care Practitioner (nurse) to join the Valentine Primary Care Network. You will work across sites, provide clinical assessment, diagnosis and treatment, and lead on patient care pathways within general practice.

The role requires 5+ years post-registration experience, non-medical prescribing, and strong clinical leadership. Pro‑ration for Extended Hours and face-to-face service across up to two sites are integral to the role.
