Job Description:

Job Summary

Integrated Care Consultant (Consultant or GP). The role supports the transformation of community services in Solihull, focusing on frail patients, reducing avoidable admissions and improving coordination across the system. Candidates should be experienced in Acute Medicine, Health Care of Older People or have significant experience managing frailty and complex patients in community pathways.

Main Duties, Tasks & Skills Required

  • Deliver high‑quality clinical care as part of multidisciplinary teams.
  • Lead pathway redesign and transformation initiatives across organisational boundaries.
  • Influence senior colleagues, build trust, and maintain delivery focus while services evolve.
  • Make autonomous clinical decisions and manage complex or long‑term conditions.
  • Process and authorise prescriptions, manage patient test results and safety alerts.
  • Collect audit data, maintain clinical records and support quality improvement.
  • Provide effective leadership within clinical pathways to maximise patient benefit and clinical efficiency.

Qualifications

  • MBBS or equivalent medical degree
  • MRCP or MRCGP
  • Full registration with GMC
  • Certification of Completion of Training
  • Inclusion on Performers List for GPs
  • Evidence of commitment to ongoing personal and professional development

  • Solihull Hospital, Lode Ln, Solihull, West Midlands, B91 2JL
  • Permanent, full‑time, band Consultant (salary £113,565.00 to £150,569.00 per annum)
