Frailty Care Coordinator – Community Health Navigator

NHS LS25/26 PCN is seeking a Care Coordinator to join the frailty team, coordinating care plans and enabling access to community services for high-risk patients. The role involves meetings with patients and carers, and collaboration across GP practices and other providers.

You will support personalised care plans, attend multidisciplinary meetings, and help families navigate services to improve health outcomes in the community.
