Job Description:

An exciting opportunity has arisen to join our specialist stroke early supported discharge team. The team covers the Merthyr Cynon and Bridgend area and will support patients who have experienced a stroke by delivering assessment, treatment and rehabilitation in their own homes to maximise independence.

Responsibilities

  • Conduct patient assessments and design individual rehabilitation plans within the patient’s home and local community.
  • Deliver therapeutic interventions in accordance with the delegated caseload of an Occupational Therapy Assistant Practitioner.
  • Coordinate care with patients, families, the multidisciplinary team and external agencies.
  • Provide support to registered Occupational Therapists and report on patient progress.
  • Participate in in‑reach activities to our acute and rehabilitation sites as required.
  • Maintain accurate clinical records and complete all required OT admin tasks.
  • Engage in supervision and in‑service training with Senior Clinicians and the Clinical Lead.

Qualifications & Requirements

  • Level 3 or Level 4 Agored qualification in Occupational Therapy Support.
  • Physical ability to move and handle patients independently and to carry and fit equipment in the patient’s home.
  • Good time‑management, prioritisation and organisational skills.
  • Excellent verbal and written communication skills, and the ability to work with patients who have communication barriers or low motivation.
  • Teamworking and interpersonal skills; ability to work effectively with patients, families and multidisciplinary teams.
  • Welsh language speaking and listening skills at Level 3 or above (desirable but not essential).
  • Competency‑based training in OT administration and team working.
  • Successful Disclosure and Barring Service check (subject to the Rehabilitation of Offenders Act, 1975).
