Job Overview

We are pleased to advertise for a community mental health Nurse/Social Worker/Occupational Therapist. We are seeking a mental health professional who wants to be at the forefront of future healthcare and integrated working within the Specialist Mental Health Services for Adults. As a multi‑disciplinary team, we provide holistic assessment and treatment pathways for people with mental health needs. Our team is made up of Clinical Psychologists, Consultant Psychiatrists, Occupational Therapists, Social Workers, and Mental Health Nurses. We also offer regular CPD sessions, reflective practice, staff support sessions, individual and group supervisions, academic training, and access to external training resources.

Responsibilities

  • Assess service users’ mental and physical health status and needs.
  • Assess other needs related to their health and wellbeing.
  • Undertake and input into risk assessments jointly with the MDT and develop robust risk management plans, in accordance with Trust policy.
  • Develop robust care plans and deliver them in partnership with multi‑disciplinary colleagues and a range of health and social care agencies, within the DIALOG+ framework.
  • Identify where best those needs can be met and facilitate signposting, onward referral or rapid access to relevant professionals and services; provide support/advice to the referring GP on effective management.
  • Provide support and advice to service users, carers and other professionals on mental health issues.

Working for Our Organisation

  • The Community Mental Health Nurse / Occupational Therapist / Social Worker will work as a member of the CMHH West.
  • The post ensures the mental health needs of adults with mental illness referred to Hillingdon CMHHs are appropriately assessed, supported and that a high level of quality care is delivered within available resources.
  • Provide support and advice to service users, carers and other professionals on mental health issues.

Person Specification

Qualifications

  • Professional Qualification in Mental Health Nursing, OT or SW.
  • Evidence of post‑basic education.

Skills and Abilities

  • Ability to participate in the work of the M.D.T. and contribute to the work of the Hub.
  • Knowledge of a range of therapeutic assessment and interventions available to reduce distress for individuals experiencing mental health issues (risk assessment, MSE, capacity assessment, and initial assessments).
  • Ability to assess and support individuals experiencing acute mental illness distress and crisis via care planning and safety planning.
  • Understanding of legislation.
  • Ability to determine priorities against conflicting demands.
  • Ability to reflect on practice and interpersonal relationships.
  • Verbal, non‑verbal communication skills and telephone skills.
  • Negotiating skills.
  • Good IT skills with evidence of Microsoft Office and SystmOne Database.

Desirable Skills and Abilities

  • Ability to teach and supervise qualified and unqualified staff.
  • Skills in a range of therapeutic interventions.
  • Skills in psycho‑social treatment options.

Experience

  • 2 years minimum post‑registration experience.
  • Experience of working with or within the community setting.

Desirable Experience

  • Supervising the work of others.
