Wellbeing Social Worker — Hybrid & Independence Focus

Stoke-on-Trent City Council is seeking a motivated Social Worker to join the Adult Social Care Wellbeing Teams. The role focuses on early intervention, prevention, and enabling independent living with a strengths-based approach across health, housing and voluntary partners.

You will deliver person-centred, outcome-focused interventions, support families and carers, and contribute to proactive care planning while managing a defined caseload in a forward-thinking team.
