The Role
There are 3 roles available:
- 37 hours per week, Permanent for South East Wellbeing Team
- 37 hours per week, fixed term 6 months for South East Wellbeing Team
- 37 hours per week, fixed term 12 months North Wellbeing Team
We are seeking a motivated, compassionate, and committed Social Worker to join our Adult Social Care Wellbeing Teams, where the focus is on early intervention, prevention, and enabling people to live fulfilling and independent lives. The Wellbeing Team plays a vital role in supporting adults using a strengths‑based approach to build on individuals’ existing abilities, networks, and community resources. Rather than relying solely on traditional service provision, the team works creatively with people to identify sustainable solutions that promote autonomy, resilience, and long‑term wellbeing. In this role, you will work closely with individuals, their families, carers, and a wide range of partner agencies across health, housing, and the voluntary sector. You will deliver person‑centred interventions that are outcome‑focused, proportionate, and tailored to the individual’s needs, helping to reduce the need for long‑term statutory involvement where appropriate. You will play a key role in empowering people to take control of their circumstances, supporting them to make informed decisions, and connecting them with community‑based support that strengthens independence and social inclusion. Key Responsibilities
- Undertake strengths‑based assessments and support planning in line with the Care Act 2014
- Work collaboratively with individuals to identify goals and build on existing strengths and networks
- Promote independence, early intervention, and prevention
- Sign post and connect individuals to community resources and services
- Manage a caseload proportionate to your experience
- Work in partnership with health professionals, voluntary sector organisations, and internal teams
- Participate in safeguarding enquiries where required
- Maintain accurate and timely case records
About You
- Is a qualified Social Worker registered with Social Work England
- Has knowledge of relevant legislation, including the Care Act 2014 and safeguarding framework
- Demonstrates a strong commitment to strengths‑based and person‑centred practice
- Has excellent communication and partnership working skills
- Is able to manage competing priorities and work effectively in a fast‑paced environment
- Shows initiative, resilience, and a proactive approach
