Job Description:

Our Community Mental Health Team supports individuals with severe and enduring mental health difficulties. This community based role requires regular travel between multiple sites to carry out duties.

Responsibilities

  • Act as the lead professional for a caseload of complex people with dementia and mental health problems.
  • Provide assessments and develop treatment plans for service users on caseload.
  • Supervise band5 staff and support workers.
  • Participate in team meetings, MDT meetings and training opportunities.
  • Provide support and advice for service users and carers.
  • Support people with complex needs and provide advice and guidance as needs change.
  • Participate in delivery of group interventions.
  • Liaise with colleagues and coordinate care for service users on caseload.
  • Support and monitor the service user whilst commencing medication.
  • Work collaboratively with other members of the MDT.
  • Attend regular supervision meetings.
  • Manage a caseload of clients with mental health conditions including dementia, acting as lead professional and providing nursing assessments and interventions on an individual and group basis.
  • Provide supervision for junior staff and students.
  • Operate within a 7‑day shift pattern between 8:00am and 9:00pm.

Qualifications

  • Appropriate professional qualification or experience at Degree Level (e.g. RMN, AHP, Social Worker).
  • Evidence of post‑registration study / continuing professional development (e.g. Mentorship in Practice).
  • Minimum of 2years post‑registration experience in mental health.
  • Evidence of leadership and management skills.
  • Experience of managing cases & caseloads and supervising staff.
  • Experience of working in partnership with other agencies and service users and carers.
  • Experience of audit/research and working knowledge of clinical governance.
  • Experience in change management and policy formulation.
  • Ability to work autonomously across the organisation and with other agencies.
  • Experience of using Incident Reporting Systems (e.g. DATIX).
  • Experience of undertaking and involvement in Serious Untoward Incident Reviews.
  • Ability to communicate effectively both verbally and non‑verbally.
  • Being responsive, flexible and seeing change as an opportunity to improve services.
  • Ability to lead the process of standard setting and audit.
  • Leadership skills and ability to work within a divisional management team.
  • Ability to generate own workload and meet strict deadlines.
  • Ability to build purposeful relationships with a range of internal and external stakeholders.
  • Proven knowledge of relevant mental health legislation and CPA ECC guidelines.
  • Demonstrate knowledge of mentorship, preceptorship and supervision and ability to provide education and supervision to junior staff.
  • Demonstrate a good knowledge of professional Codes of Conduct and staff responsibility to identify gaps in knowledge.
