Care Transition Coordinator for Older Adults

Community Care Live is seeking a Discharge Flow Coordinator to support older adults returning home from hospital. You will coordinate journeys through Short-Term Services, build strong relationships with colleagues, partners, families and individuals, and help ensure timely, person-centred discharges.
