Overview
We are looking for an enthusiastic Social Worker to join our West Follow-On Team, based at Priory House in King’s Lynn. This is a varied and rewarding role supporting people following discharge from hospital, whether they are returning home or moving into care placements.
Responsibilities
- Manage your own caseload.
- Undertake Care Act assessments.
- Complete Mental Capacity assessments.
- Lead Section 42 safeguarding enquiries.
- Make confident decision-making, strong partnership working and implement a person-centred approach.
Qualifications
- You must be registered with Social Work England as a practising Social Worker.
