Sanctuary Personnel in Herefordshire is seeking a committed Social Worker to join the City Adult Locality Team on a permanent full-time basis. You will conduct assessments, develop care plans and coordinate with health and housing partners to support adults in the community.
You will apply the Care Act 2014 and other relevant legislation, complete safeguarding enquiries, and contribute to hospital discharges and timely care coordination.
