Palliative Care Social Worker — Home & Hospice Support

St Raphael’s Hospice invites applications for a Social Worker to provide social, psychological and practical support to patients and their families. A minimum of 50% of duties involve direct client work, within hospice and home care settings where appropriate.

Essential requirements include a Degree in Social Work, registration with Social Work England and significant post-qualification experience in multidisciplinary teams, with a focus on loss and grief in lifelimiting illness.
