Main area: Community Integrated Response. Contract: 12 months (Fixed term). Hours: Part‑time – 18 h/wk, worked across 7‑day rota, Monday to Sunday 08:00–20:00 shift patterns.
Site: St Catherine’s Health Centre, Birkenhead. Salary: £39,959 – £48,117 pro‑ratum per annum. Closing date: 21/07/2026 23:59.
Job overview
Join the Urgent Community Response Team, based with the Community Integrated Response Team. The role covers Frailty Virtual Wards, Urgent Community Response and Home First across extended hours 7 days per week (08:00–20:00). Coverage is roster‑based and includes weekends.
Main duties
• Perform comprehensive assessments for patients with physical, mental health and complex social needs.• Implement treatment plans and collaborate with multidisciplinary teams.• Liaise and refer into wider health, social care and voluntary agencies to achieve best outcomes.
• Contribute to service improvement initiatives; attend in‑service training and continuous professional development; supervise and support staff where required.
Experience & Knowledge
- Broad experience in community rehabilitation and a range of procedures/interventions.
- Up‑to‑date portfolio of continuous professional development.
- Understanding of the clinical governance agenda.
Skills & Attributes
- Excellent time management and prioritisation.
- Highly developed organisational skills.
- Ability to work under pressure and respond quickly to unpredictable caseloads.
- Capacity to manage, supervise and motivate staff.
Personal Qualities
- Flexible and punctual approach to work.
- Autonomous practitioner with strong empathy for patients and families.
- Valid UK driving licence and vehicle access for home visits.
Qualifications and Professional Training
- Occupational Therapy degree or equivalent qualification.
- Registered with the Health Professions Council (HPC) as an Occupational Therapist.
- Current HCPC registration and ongoing professional training.
