Community Link Worker: Pathways & Wellbeing

NHS Lanarkshire is recruiting a Community Link Worker to join a Primary Care Multi-disciplinary team. You will work with GP practice patients to identify barriers and support decision‑making to live well.

The role is 36 hours per week over Monday to Friday, core hours 08:00–18:00, fixed term to 30 September 2027, with secondment option for internal staff. You will build pathways, improve health and well‑being, help patients access community services, and promote the CLW role within Primary Care
