About the Role

The post holder will hold a certificate of Qualification in Social Work, Diploma in Social Work, DIPSW, or Social Work Degree and will be fully registered under Social Work England Registration Body.

The role of a Social Worker involves working with people—including those with physical disabilities, learning disabilities, and mental health needs—and their carers in their own environments to provide support with their care needs and help them attain an agreed level of independence.

You will work with care agency staff, occupational therapists, physiotherapists and social workers, develop and monitor reablement programmes, and undertake Safeguarding and Section 42 inquiries as part of a multi‑disciplinary team.

As part of a multi‑disciplinary team you will ensure client and carer wishes are central to decision making, formulate and implement appropriate care plans, maximise their independence, and help them remain living in the community or move seamlessly into more suitable accommodation.

Qualifications

Must hold a certificate of Qualification in Social Work, Diploma in Social Work, DIPSW, or a Social Work Degree and be fully registered under Social Work England Registration Body.

  • Formulate and implement care plans that maximise independence and facilitate community living or suitable accommodation transitions.

Appointment Details

This is a permanent position and appointment to this post will be subject to the outcome of an Enhanced Disclosure obtained through the Disclosure and Barring Service (DBS).
