About the role
Staffordshire County Council’s Provider Services team is looking for Support Facilitators to deliver person-centred reablement care in the community. The service runs 7:00 AM to 10:00 PM, with shifts between 7 AM and 10 PM, including weekends. Applicants must live within 30 minutes of our base location (ST5 1BL) and be able to travel within the community, primarily Newcastle under Lyme Borough. A full UK driving licence and access to your own vehicle is essential.
Key responsibilities
- Provide person-centred reablement care in people’s homes, respecting dignity and promoting independence.
- Support individuals with daily living tasks, personal care, and medication administration.
- Communicate effectively with individuals, families, and colleagues to build positive relationships.
- Respond flexibly to changing needs and embrace digital tools to improve service delivery.
- Maintain accurate records and comply with health, safety, and safeguarding standards.
- Undertake manual handling tasks as required.
- Drive (using your own vehicle) to deliver care across the local area.
- Complete interventions using Assistive Technology.
Requirements
- Demonstrated caring and compassionate nature with a genuine desire to make a positive difference.
- Strong communication skills and ability to build trust and positive relationships.
- Flexibility and resilience, adapting to changing needs and working patterns.
- Practical skills and reliability, including a full driving licence, physical ability for care tasks, and commitment to learning and development.
- Experience in social care (NVQ Level 2 or equivalent). (NVQ2 Provided.)
- Driver with access to a vehicle for work purposes.
- Caring, patient, and committed to empowering people to live independently.
- Ability to work flexibly across days, evenings, weekends, and bank holidays.
- Physically able to support personal care and manual handling tasks.
- Excellent communication with a positive outlook and ability to stay calm under pressure.
- Living within 30 minutes of our base location.
Additional information
This role is regulated by the CQC and requires full employment history from leaving secondary school. A pre‑employment health assessment and an enhanced DBS check are required. The position is not eligible for sponsorship due to recent legislation changes.
