Social Worker (Adult Social Care)

Overview

Take on a variety of adult social work tasks, including Care Act Assessments, person‑centred support planning, mental capacity assessments, and Section42 enquiries. Work with a strength‑based approach to empower individuals and champion independence, and engage in crucial CHC and s117 work to ensure seamless support. You will collaborate with partner agencies and have opportunities for ongoing professional development through regular supervision and funded specialist training.

Responsibilities

  • Conduct Care Act assessments and develop bespoke support plans for adults with diverse needs.
  • Carry out mental capacity assessments and manage Section42 enquiries.
  • Collaborate with partner agencies to deliver holistic care and ensure continuity of support.
  • Provide regular supervision to colleagues and participate in professional growth opportunities.
  • Adhere to safeguarding policies and promote the welfare of children, young people and vulnerable adults.

Qualifications & Experience

  • Registered Social Worker with Social Work England.
  • Relevant university qualification in social work.
  • Up‑to‑date knowledge of current legislation and guidance.
  • Experience in person‑centred assessment, care management and working with adults who have physical or learning disabilities, mental health needs, and older adults.
  • Drive to pursue further qualifications such as Best‑Interest Assessor, Approved Mental Health Professional, or Practice Education.
  • Full, clean driving licence and willingness to cover a vibrant county.
