Senior Social Worker: Hospital Discharge Lead & MDT Champion

LB RICHMOND UPON THAMES & LB WANDSWORTH is seeking a Senior Social Worker to join the Richmond RRRT hospital discharge team. You will lead complex discharge planning, oversee hub management, and champion strengths-based practice to deliver safe, timely, person-centred outcomes for residents.
