Community Wellbeing Link Worker

Sowe Valley Primary Care Network in Coventry is seeking a Social Prescribing Link Worker to help people improve health and wellbeing by connecting them with community services and practical support.

You will collaborate with GP practices within the PCN, support individuals and families to live independently, and address wider determinants of health such as debt, housing and loneliness, with a multidisciplinary team backing your work.
