Urgent Community Response Nurse – Palliative & Home Care

The East of England Community Health and Care NHS Trust is seeking an Urgent Community Response Nurse to support unplanned care at patients’ homes from Norwich Community Hospital Hive. The role involves holistic assessment and rapid care planning within a multidisciplinary team.

You will work across a 7-day week, including weekends, 8am-8pm, with duties in palliative and end-of-life care, wound and catheter care, and urgent community response. Driving licence and vehicle access are essential.
