Urgent Community Response Nurse

Opportunity for a registered nurse with skills in palliative / end of life care, wound care, catheter care and urgent community 2 hour response for patients and a strong desire to further develop your community nursing skills and experience.
You will be required to work over a 7 day working week including weekends and will be hard working, motivated, with excellent communication skills. You will be able to demonstrate knowledge and skills in holistic assessment, to demonstrate that you use your initiative and have a keen interest in working with this group of patients. You must hold a full and valid driving licence and have access to a vehicle.
You will support staff working at a band 3 and 4 level, along with students and apprentices.
Due to the unplanned nature of the role we would recommend newly qualified staff to not apply.
Main Duties and Responsibilities
- Work within the Urgent Community Response Team to prevent unnecessary admission to hospital.
- Assess and prescribe care pathways for patients in crisis situations with long-term conditions, frail & elderly, palliative and end-of-life care and/or rehabilitation needs, to achieve quality of life and independence where possible.
- Act as the patient’s advocate, facilitating choice and patient empowerment.
- Provide evidence-based clinical/therapeutic interventions based on best practice.
- Support early discharge from hospital.
- Work with all health care professionals, statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
- Triage referrals on a rotational basis.
Clinical Expectations
- Assess, plan, implement and evaluate all aspects of patient care, and develop treatment/care pathways that may be delivered from a range of options.
- Follow a holistic assessment, devise an individualized care pathway for each patient, modifying it as required.
- Be aware of assistive technology and utilize where required.
- Communicate complex and sensitive information to patients and carers regarding assessment, diagnosis, prognosis and treatment plan.
- Make adjustments for patients with hearing, mental capacity or other comprehension barriers to gain understanding, consent and concordance.
- Establish and maintain therapeutic relationships with patients and carers, incorporating motivation, encouragement and confidence building to enable engagement in treatment/care/management plan.
- Liaise with GPs, Social Services, inpatient teams, other allied health professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately.
- Undertake risk assessments and act upon them appropriately.
- Participate in multidisciplinary/multi- agency meetings as appropriate, e.g. Gold Standard Framework.
- Ensure activity data and clinical information is recorded on SystmOne and completed on a daily basis.
- Demonstrate dexterity and coordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.
- Order equipment and supplies via the electronic ordering systems used within the team.
- Ensure informed consent is obtained prior to initiating interventions.
- Adhere to infection control policies and universal precautions when exposed to bodily fluids, infected material or blood products.
About Us
The post can be based at the new dedicated district nursing “Hive” building at the Norwich Community Hospital. You will be working closely with other professionals in the urgent community response and virtual ward as well as linking with the planned community nursing teams. This dynamic and busy team requires keen and motivated nurses to provide assessment and quality interventions, in particular unplanned care to the population of patients restricted to receiving care in their homes.

Person Specification
Qualifications
- Diploma or 1st Level Degree
- RN Qualification
- Current NMC Registration
- Evidence of Continuing Professional Development
- Assessor/mentorship Preparation; ENB 998 or Equivalent
Experience
- Evidence of working within the community or acute setting
- Attendance at in-house training and short courses

- This is a 7 day a week service and some weekend working will be expected. Hours of service delivery, 8am-8pm, 365 days per year.
