Service Change Lead, Health & Care Transformation (Hybrid)

Sussex Community NHS Foundation Trust is seeking a Service Change Partner to support the design and delivery of service improvements and ensure excellent care at the heart of the community. The role involves working with professionals across health and social care in Sussex, and will contribute to an approved programme of works.

This is a hybrid role based around home working and Brighton General Hospital, with a minimum of 3 days per week for part-time colleagues; full-time also considered.
