Part-Time Community-Based Family Health Practitioner

HCRG Care Group is seeking a Health Family Practitioner to join the Lancashire Young People & Healthy Families Service. You will support preschool and primary‑aged children and their families under the direction of Health Visitors, School Nurses and Staff Nurses in community settings.

This is a part‑time role (30 hours per week) offering development opportunities within a multidisciplinary team and a commitment to compassionate, community‑based care across Lancashire.
