Job overview

This is an a2PA post, in palliative medicine working in hospital and community settings to cover a one year period of maternity leave.

As a key member of the multi professional team you will work as part of the innovative ‘Connected Palliative care’ service which provides specialist palliative care, 24/7 rapid response home care for dying patients and a dedicated central coordination hub which manages our end-of-life care register.

The post will be between Oldbury Council House & Midland Metropolitan Hospital, and the post holder will be required to work at any of the Trust’s inpatient sites and carry out domiciliary visits.

This post for 12 months will start on September 1 st.

The post holder will work alongside the established consultant team to:

  1. Provide specialist palliative care medical care to people in our hospitals and community footprint.
  2. Take a leadership role in the Palliative and End of Life Care Quality Improvement Program building on established work with acute and community specialities to deliver and further improve specialist palliative and end of life care patient and carer experience across all disease pathways.
  3. Support as needed selected cancer & non-cancer MDT’s & board rounds.
  4. Be supported to develop subspecialist interests if aligned to the service strategy.
  5. Deliver & design education and training as needed.

Information about the Specialist Palliative Care Service

The innovative ‘Connected Palliative care’ service which provides specialist palliative care, 24/7 rapid response home care for dying patients and a dedicated central coordination hub which manages our end-of-life care register. Specialist in-patient hospice care is sub contracted to two local partner hospices.

  • Entry on Specialist Register in Palliative Medicine or possession of a CCT in Palliative Medicine or substantial experience in a senior medical post in Palliative Medicine or within 6 months of CCT.
  • MRCP or equivalent.
  • Higher Degree

Clinical Experience

  • Ability to take full and independent responsibility for the management of complex palliative care patients.
  • Ability to take full and independent responsibility for the care of patients
  • Special interest that complements the existing consultants

Clinical Effectiveness

  • Experience of leading successful quality improvement activity.
  • Ability to use the evidence base and clinical audit to support decision-making.

  • Better protect elective / planned care service delivery across the system by organising on a ‘hot and cold’ arrangement
  • Consider the optimal use of resources through consolidation and specialisation, and
  • Seek to reduce unwarranted variance, level up / standardise care through a ‘Networked Service Solution’ approach across the four sites
