Age Well Coordinator — Home Visits & Community Support

Age UK Northamptonshire is seeking an Age Well Coordinator for Northampton, joining a multi‑agency team to coordinate personalised, community‑based support for older people.

You will visit clients at home, complete holistic assessments, make onward referrals, provide practical help, and work with GP practices and community teams to reduce loneliness and promote wellbeing in local neighbourhoods. Hybrid Northampton PCN location.
