Overview
The role has become available to join a committed and motivated Patient Flow Team who work hard to deliver excellent standards in a positive working environment. The Hospital Team works in partnership with individuals to identify what matters to them, including undertaking Mental Capacity Assessments, Continuing Health Care Assessments, and leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital.
We work alongside a range of professionals within the teams, including colleagues from the health board, therapies and external providers. It is an exciting time to join adult social care with friendly, experienced and supportive colleagues who are dedicated to providing a service that puts citizens at centre of our work.
Responsibilities
- Undertake Mental Capacity Assessments, Continuing Health Care Assessments and lead Best Interest Meetings.
- Ensure patients have a safe and timely discharge from hospital.
- Work collaboratively with professionals within the health board, therapies and external providers.
Qualifications
- Social work qualified and professionally registered with Social Care Wales.
- Knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, including outcome‑focused assessment and care planning.
- Knowledge and working practice of Continuing Health Care, the Mental Capacity Act 2005 and the Deprivation of Liberty Safeguards.
