Senior Social Worker – Hospital Discharge Lead

Lancashire County Council is seeking a Senior Social Worker to support hospital discharge processes within the Adult Social Care service. This role focuses on safe, timely, person-centred discharge planning, working closely with health colleagues, families and partners to move people to the most appropriate setting.
