Specialist Pharmacist – Frailty & Elderly Care (Inpatient)

Milton Keynes University Hospital NHS Foundation Trust is seeking a Specialist Pharmacist – Frailty and Care of the Elderly to work with the Pharmacy team to develop specialist clinical services for older adults, focusing on safe discharges and transfer of care.

The post holder will deliver comprehensive, patient‑centred geriatric pharmaceutical care in newly built wards, collaborate with frailty teams, and support care across the elderly pathway within a Trust rated Good by the CQC.
