PCN Proactive Care Admin Assistant: Care Team Coordinator

Tone Valley Primary Care Network is seeking an Administration Assistant to join our Taunton-based PAC, EHCH and CIH teams. The role focuses on delivering broad administrative support, patient data handling and efficient diary management to enable proactive care services across five GP practices.

The successful candidate will work with Operational Leads and partner practices, supporting information dissemination, meeting administration and service development across the neighbourhood.
